Here are the requirements:
🔸 For new registration, transfer & reactivation
▪️ Student ID or Library Card
▪️ Employee’s ID
▪️ Any Government Issued ID
🔸 For change/ correction of entries
▪️ Birth Certificate (PSA copy) – registration record contains erroneous entries including wrong or misspelled name, birth date, birth place or typographical error
▪️ Marriage Contract (PSA copy)/ Court Order – change of name by reason of marriage or by court order
